
Our Services
At Sierra Peak Health Inc., we provide comprehensive hospice services, including personalized nursing care, physician collaboration, emotional and psychosocial support, and assistance with personal care. We also offer medication coverage, medical equipment, respite care for caregivers, and volunteer companionship. Our goal is to ensure comfort, dignity, and quality of life for our patients and their families.

Nursing Services
A hospice patient is assigned a nurse case manager, who typically visits one to three days a week depending on the patient's need. Hospice patients and their caregivers can also access an on-call nurse 24 hours a day.

Physician Services
A hospice patient's regular physician often provides care, in cooperation with a hospice medical director. The physician can be contacted through the nurse case manager.

Hospice Aide/
Certified Nursing Assistant
These aides can help patients with their personal care and typically visit two to three times per week depending on the patient's need. They follow a plan of care created specifically for the patient by the nurse case manager.

Medical Equipment and Supplies
Hospice provides the equipment necessary to provide a safe, comfortable, caring environment in the patient's home. These supplies might include, for example, a hospital bed, a wheelchair, and oxygen, as well as adult diapers, underpads, and gloves.

Respite Care
This form of temporary, short-term assistance can help alleviate or avoid caregiver burnout and stress once every certification period.

Volunteer Services
A Hospice volunteer will provide companionship and educational resources to hospice patients when needed. The volunteer will provide a listening ear and a hand to hold for a patient who needs, per request. Volunteer, however, does not assist in any housekeeping and physical care or the patient.
Medical Social Service​s
Terminally ill patients are assigned a medical social worker to assist them with their emotional and/or psychosocial needs.

Counseling Services
Hospice patients and/or their loved ones might need dietary services, as appropriate; spiritual support; and bereavement counseling for family and caregivers after the patient's death.​
​
Medications
Hospice typically covers all medications related to the hospice terminal diagnosis and those intended to control or alleviate pain and symptoms.​
